At Rainbow Hill Recovery, we are dedicated to making LGBTQ+-affirming care accessible to everyone. We accept most out-of-network insurance policies and are actively expanding our list of In-Network providers. As of October 1, 2024, we are proud to be In-Network with Blue Shield of California, and we’re working on securing more In-Network contracts to increase accessibility for our clients.
Understanding Insurance: HMO vs. PPO
Navigating insurance options can be complex, but understanding the differences between HMO and PPO plans will help clarify your coverage.
HMO
Health Maintenance Organization
HMO plans typically require you to select a primary care physician (PCP) and obtain referrals to see specialists. You must use innetwork providers for your care, and out-of-network services are generally not covered unless there’s an emergency.
PPO
(Preferred Provider Organization)
PPO plans offer more flexibility in choosing healthcare providers, allowing you to see both in-network and out-of-network providers without needing a referral. While staying in-network lowers your out-of-pocket costs, out-of-network services are still covered at a lower rate, providing more options for your care.
At Rainbow Hill Recovery, we accept most PPO plans for out-of-network benefits and will assist you in maximizing your insurance coverage. Our admissions team can verify your benefits and explain how your insurance works for our programs.
Our Programs and Pricing
Partial Hospitalization Program
Insurance Rate: $20,000/month
Flat Fee Rate: $10,000/month
Intensive Outpatient
Insurance Rate: $13,000–$15,000/month
Flat Fee Rate: $4,500–$6,500/month
Important Notes
- Once your out-of-pocket maximum is reached, insurance typically covers 100% of their allowable amount.
- Any “patient responsibility” shown on an Explanation of Benefits (EOB) will not be collected if it exceeds our flat fee.
- Our flat fee structure provides peace of mind and financial predictability for every family we serve.

Flat fee rates represent the maximum patient responsibility and supersede any insurance balance billing. Families will never be charged beyond the flat fee amount, regardless of what may appear on insurance statements.
